Diagnostic Value of Peritoneoscopy



Abstract
Although there are many diagnostic procedures for various diseases of hepato-biliary system, most of them cannot escape being handicapped by one or the other difficulties. The peritoneoscopy eliminates some of these difficulties and is the method commonly utilized. Its greatest advantage is in the direct observation of the lesions in the peritonea! cavity and in the selection of a particular lesion for the histological examination.
It is the purpose of this study to report the results of the 21 cases examined at the Pusan National University Hospital and to evaluate the results. A brief review of the literature on this procedure is also made.
-Results and Comments
Of the 21 patients studied, the majority was male (17). The age ranged between 21 and 67. The clinical impressions on these patients before the examinations varied from unknown to liver cirrhosis, hepatic tumor or tuberculous peritonitis (Table).
As seen in the table the diagnosis through the peritoneoscopy have been modified considerably or confirmed unequivocally the impressions previously made. The gross inspection of the lesions in the peritioneal cavity allowed to detect many insignificant lesions which might have been missed by the ordinary laboratory procedures. The diagnosis made through the peritonescopy were invariably confirmed by the histologic diagnosis later. Naturally the selecction of a suitable lesion made the histologic diagnosis more accurate. No serious complication had occurred during or after the peritioneal inspection.
From the experiences made through the peritoneoscopy, it is concluded that the peritoneoscopy, should be recommended as the method of choice in making the final diagnosis of the henato-biliary pathology. Importance of other tests are not denied, however, when these tests failed to clarify the exact nature, then the perituneoscopy may be of a great help.
